ABEM managing conflict cases, explained
A managing-conflict case is one of the communication cases on the ABEM Certifying Exam. Someone is pushing back, and you have to reach a safe outcome. Here is what it is testing and how to practice it.
What the case looks like
There is a source of friction to work through
The case gives you someone pushing back — a reluctant consultant, a patient who wants to leave against medical advice, or an upset family member. Your job is to move toward a safe outcome without the situation escalating.
It tests de-escalation and negotiation
The credit is in how you lower the temperature and find common ground: acknowledging the other person’s position, staying calm and professional, and advocating for the patient without turning it into a fight.
Keep the patient’s safety at the center
However the conversation goes, the anchor is what is safe for the patient. Strong examinees are flexible on tone but firm on the medicine that actually matters.
Have a next step, not just a stance
A good outcome is concrete: what you and the other party will do now. Naming a clear, safe plan — and documenting or escalating appropriately — is part of what the case looks for.
Common traps
- Matching the other person’s hostility instead of de-escalating it.
- Caving on the patient’s safety just to end the conflict.
- Digging in and turning a negotiation into a standoff.
- Ending without a concrete, safe next step.
